We're on the hunt for a Care Coordinator to join us at our Supported Living service in Eastleigh. This service is a welcoming and friendly environment with a dynamic team that supports a fantastic group of people!

As a Care Coordinator with us you'll play a pivotal role in ensuring the people we support are kept safe by ensuring our care delivery colleagues are deployed effectively and that all administration is undertaken in line with relevant regulations and legislation. You'll be responsible for the managing and coordinating of the care delivery team, you will play a key role in ensuring that the relevant documentation is in place for any new people we support, discharges and transfers. You'll conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational requirements of our care colleagues. You will also work alongside the management team to coordinate with families, healthcare professionals and external agencies, handle enquiries, schedule appointments and meetings and handle all information with confidentiality.
We're interested in our Care Coordinators having both the right experience and the right values. This is why at this service our Care Coordinators should have experience working in Health & Social Care, have experience in the use of Microsoft Office and be computer literate, have prior experience in an administration role, and have experience providing excellent customer service.

This is why at this service our Seniors should have previous experience as a Senior Support Worker and a Level 3 in Health & Social Care as well as experience working with individuals requiring enhanced support, including behaviour's of distress, mental health conditions, acquired brain injury, autism, forensic histories and people who have experienced trauma.

* Image new laptops with Windows 11.
* Arrange appointments with users to replace legacy devices, including transferring essential data (kept to a minimum where possible).
* Provide a full handover with each user, ensuring everything is working as expected (checklist will be provided).
* Be self-driven and organised to manage and complete device replacements efficiently.
* Attend clinical/public sector sites to manually upgrade devices to Windows 11 where needed.

* Lead on the delivery of end-to-end procurement processes across a diverse portfolio including services, works, and concessions

The ideal candidate will have prior experience working within a Local Authority and can demonstrate knowledge of Adult Social Care matters.

Duties may include:

* Undertake all aspects of legal work relating to Adult Social Care Legislation, including, but not limited to, Court of Protection, Section 21a, Section 16 and Designation matters

* Liaise and work collaboratively with other legal representatives

* To prepare and manage a varied caseload in relation to all aspects of Adult Social Care matters

* Represent the Council in court and may need to conduct advocacy when required

* Deliver an Estates and Facilities service which is proactive in its approach to need and the Estates & Facilities Strategy
* Implement policies and procedures relating to the buildings and FM services
* Manage building and compliance audits and inspections across all areas of responsibility against contract deliverables
* Manage, maintain and develop all statutory records for asset management, compliance and planned preventative maintenance (PPM)
* Complete regular and recorded site inspections covering, but not limited to, cleaning audits, condition audits and compliance
* Liaise with suppliers of services for both planned and reactive tasks to be undertaken with minimal disruption to normal operations and are completed to the time, cost, quality or contractual requirements
* Manage own subcontractors to ensure all work is completed to time, cost and quality requirements
* Lead all project works from the Estates & Facilities operation support local management where necessary
* Arrange quotes for all works out of scope of contracted services including capital and one off purchases for all sites
* Ensure all financial procedures are followed for Estates & Facilities works
* Liaise with and support Contractors providing third party PPM and compliance services across the estate, providing scrutiny over reports and documentation provided under those contracts Health, Safety and Environment
* To support and work with the Health and Safety Adviser to maintain safe working practices and a proactive safety culture throughout the organisation
* To ensure all sites are compliant with all statutory obligations in relation to Health and Safety
* Support the Head of Estates & Facilities and Operations Coordinator with the day to day management of vehicles and fleet management including vehicle maintenance
* Proven experience in a similar role and able to hit the ground running, experience managing a team of c.10 Technicians * You must hold a full clean driving licence to be considered for this role
