Cheshire West and Chester Council is looking for an experienced Qualified Social Worker to join the Hospital Discharge Team. This progressed role supports hospital discharge planning and post-discharge assessments in care homes, ensuring safe and person-centred transitions for adults.

You will play a key role in supporting Cheshire West residents within acute hospital settings, undertaking assessments and working closely with health colleagues, service users, and families to achieve positive outcomes.

Key Responsibilities:

Support safe and timely hospital discharges.

Undertake assessments in care homes following discharge.

Apply knowledge of Continuing Healthcare (CHC) processes and support joint decision-making.

Complete face-to-face assessments on hospital wards as required.

Work across multiple acute hospitals, including the Countess of Chester (primary base), Arrowe Park, Warrington, Leighton, and other external Trusts.

Maintain accurate case recording and evidence-based practice.

Manage caseload effectively while working within a multi-disciplinary setting.

Hybrid & Flexible Working:

Ward-based assessments required.

Hot-desking available at several sites: Countess of Chester, Leighton Hospital, Wyvern House (Winsford), The Portal (Ellesmere Port), and Lightfoot Lodge (Chester).

Flexible working patterns supported in line with service needs.

Requirements:

Qualified Social Worker with Social Work England registration.

Experience in hospital discharge and adult social care.

Strong knowledge of CHC and discharge processes.

Excellent assessment, recording, and risk management skills.

Ability to work flexibly across hospital and community settings.
