We are seeking a Qualified Social Worker (QSW) to join our Integrated Neighbourhood Teams. This role involves working collaboratively to deliver high-quality social care assessments, reviews, and interventions, supporting adults in the community.

Key Responsibilities:

Conduct Care Act assessments and reviews.

Undertake Mental Capacity Act (MCA) assessments and Safeguarding investigations.

Complete Single Funding Gateway (SFG) assessments and support processes for Continuing Healthcare (CHC).

Work collaboratively within an integrated multidisciplinary team.

Requirements:

Qualified Social Worker (QSW) with HCPC registration.

Proven experience in Safeguarding, Care Act assessments, and CHC processes.

Strong communication, organisation, and decision-making skills.

Ability to work both independently and as part of a team.

Pay Rate: £30 per hour
Work Pattern: Hybrid - 2-3 days per week in the office
