We are seeking an experienced Qualified Social Worker to join the Hospital Discharge to Assess South Team in the Wythenshawe area of Manchester. This role focuses on supporting timely hospital discharges through Care Act assessments and post-discharge reviews, ensuring citizens receive the right long-term support before transitioning to community teams.

Key Responsibilities:

Carry out Care Act assessments and timely reviews of citizens recently discharged from hospital.

Review and evaluate packages of care put in place by the hospital team.

Assess for long-term support needs before transferring cases to community teams.

Complete Mental Capacity assessments, Best Interest decisions, and address safeguarding concerns where required.

Work in a strengths-based, person-centred way, promoting independence and wellbeing.

Hybrid working, balancing remote work with in-person visits.

Act independently, seeking advice on complex cases when necessary.

Focus solely on clearing outstanding reviews - no duty function required.

Requirements:

Qualified Social Worker with Social Work England registration.

Experience in hospital discharge and community-based adult social care.

Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.

Ability to manage workload independently and work to tight deadlines.

Excellent communication, assessment, and report-writing skills.

Contract Details:

Initial term: 3 months, with potential for extension.

Hybrid working arrangement (some site visits in Wythenshawe).
