Qualified Social Worker - Initial Contact Team
Location: Lambeth Civic Centre
Pay Rate: £32.52 p/h
Start Date: ASAP

We are seeking an experienced and dedicated Qualified Social Worker to join Lambeth's Initial Contact Team. This role requires office attendance to provide essential support in undertaking Care Act and Mental Capacity assessments, and determining Section 42 thresholds for adult safeguarding concerns.

You will cover duty at the Civic Centre and carry out a variety of key responsibilities, including:

Key Responsibilities:

Manage a caseload involving vulnerable adults and/or individuals with complex needs, taking urgent action where required to protect them and others in emergency situations.

Undertake timely and appropriate assessments of social care needs in collaboration with health and partner agencies, taking into account the wishes of individuals and their carers.

Plan and implement tailored responses to meet identified needs in partnership with relatives, carers, advocates, and relevant professionals.

Ensure services are delivered within available financial resources to meet assessed needs.

Coordinate, monitor, and review care plans, identifying service gaps and informing line management.

Maintain accessible costed resource information for service users, carers, and colleagues.

Job Activities:

Respond professionally to requests for assistance, gathering sufficient information to determine assessment requirements.

Maintain accurate and up-to-date case records in line with national and local policies.

Produce professional reports to inform management decision-making.

Notify the Line Manager of unmet needs due to resource or budget constraints.

Operate within budget and follow Council financial regulations and policies.

Assessment Duties:

Lead on needs assessments, coordinating involvement from other professionals where necessary.

Focus on both strengths and areas for improvement during assessments.

Ensure service users and carers are fully engaged in the assessment process.

Liaise with specialists and partner agencies to inform assessment outcomes.

Care Planning Duties:

Negotiate with service users, carers, and stakeholders to agree on how identified needs will be met.

Create clear, goal-driven care plans specifying service aims and provider responsibilities.

Provide advice, support, and guidance to service users and carers, including access to counselling, translation, interpretation, or advocacy services.

Requirements:

Qualified Social Worker with Social Work England registration.

Experience in adult safeguarding, Care Act, and Mental Capacity assessments.

Strong understanding of Section 42 safeguarding procedures.

Ability to work collaboratively with multi-agency partners.

Excellent written and verbal communication skills.
