Job Advert - Locum Social Worker
Location: Bromley - Hayes and Five Elms Locality Team
Rate: £32 p/h
Contract: 3 months (potential to extend)
Start Date: 18th August - non-negotiable

We are seeking an experienced Social Worker to join the Hayes and Five Elms Locality Team in Bromley.

The team focuses on:

Ongoing care management interventions

Annual reviews of support plans

Extra care housing support

Direct payment work

Assessments under the Care Act 2014

Supporting young people transitioning from Children's to Adult Services

You will support younger adults with physical disabilities and sensory impairment, as well as older people, to live as independently as possible, working with their carers to manage risks appropriately. This role requires strong communication and negotiation skills, the ability to work within a multidisciplinary framework, and building strong relationships with partner agencies.

We are looking for someone qualified, motivated, enthusiastic, innovative, and a great team player.
