Our IUCT provides short-term, wraparound care support for people in their own homes for up to 72 hours, helping to prevent hospital admissions and supporting timely hospital discharges. You will play a pivotal role in shaping and delivering high-quality, person-centred services that meet the needs of our community.
About the Team
The Community Integrated Urgent Care Team consists of:
Social Workers
Assessors
Physiotherapists
Occupational Therapists
Assistant Practitioners
Care Assistants
Key Responsibilities
Lead and manage a multidisciplinary team to deliver timely, effective, and safe urgent care interventions in the community.
Drive forward service transformation plans, ensuring innovative, integrated, and outcome-focused approaches.
Work collaboratively with health colleagues, local authority teams, and other system partners to achieve shared goals.
Monitor performance, ensuring compliance with statutory duties and best practice standards.
Support staff development, wellbeing, and high-performance culture.
Requirements
Proven management experience in adult social care or health services.
Strong leadership skills with the ability to inspire and motivate multidisciplinary teams.
Experience of partnership working with health services and other agencies.
Excellent communication, organisational, and problem-solving abilities.
Relevant professional qualification (e.g., Social Work, Nursing, Occupational Therapy, Physiotherapy) and registration with the appropriate body.
