Newcastle City Council is seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Learning Disabilities and Autism (LDA) Team. You will be part of a supportive, multidisciplinary environment working to deliver high-quality care and support for adults with learning disabilities and/or autism, many of whom also experience complex mental health needs.

This is a vital frontline role where you will assess needs, co-produce care plans, and ensure the safety, wellbeing, and independence of some of the city's most vulnerable residents.

Key Responsibilities:
Undertake Care Act 2014 assessments, risk assessments, and safeguarding investigations

Co-produce person-centred support plans with service users and their families

Work collaboratively with health professionals, housing providers, advocacy services, and other stakeholders

Support hospital discharges and transitions into community-based services

Maintain accurate, timely, and professional records using the Council's case management system

Apply knowledge of legislation including the Mental Capacity Act, Care Act, DoLS, and safeguarding frameworks

About You:
Qualified Social Worker with Social Work England registration

Demonstrable experience working with adults with learning disabilities, autism, and/or mental health needs

Strong knowledge of relevant legislation, guidance, and good practice

Confident in handling safeguarding concerns, capacity assessments, and multi-agency working

Commitment to inclusion, rights-based practice, and promoting independence

Excellent communication, organisational, and analytical skills
