We are seeking an experienced Hospital Social Worker to join the hospital discharge teams at Hull Royal Infirmary and Castle Hill Hospital. This role offers flexible working hours, to be agreed between you and the manager, providing a great opportunity for work-life balance while contributing to a busy and supportive team.

Key Responsibilities:

Conduct timely hospital discharge assessments and manage cases through to safe discharge

Work collaboratively with NHS colleagues and wider adult social care teams

Support patients and families with care planning, safeguarding, and navigating relevant health and social care pathways

Maintain up-to-date, accurate case records and adhere to statutory duties

Essential Requirements:

Qualified Social Worker with Social Work England registration

Recent experience working in a hospital-based setting

Must be able to travel to both Hull Royal Infirmary and Castle Hill Hospital

Strong working knowledge of the Care Act, hospital discharge processes, and Continuing Healthcare (CHC)

Additional Information:

The post is initially temporary, with the potential to extend up to 48 weeks for the right candidate

Flexibility is offered around working times, subject to agreement with the team manager

On-site presence at both hospital sites is essential
