We are currently seeking a qualified and experienced Hospital Social Worker to join the Hospital Discharge Team at Barnsley Hospital. This is a temporary 12-week position, offering hybrid working with a minimum of 3 days per week on-site, with flexibility for additional on-site work as required.

Key Responsibilities:

Conduct timely assessments and discharge planning for patients admitted to Barnsley Hospital

Work in partnership with health professionals to ensure safe and effective hospital discharges

Undertake visits across the borough of Barnsley as needed, sometimes at short notice

Apply sound knowledge of relevant legislation, policies, and procedures including the Care Act

Maintain accurate case recordings and meet statutory requirements

Essential Requirements:

Qualified Social Worker with Social Work England registration

Recent experience (within the last 12-18 months) in a hospital-based social work setting

Strong knowledge of hospital discharge processes, Continuing Health Care, and the Care Act

Ability to work independently, manage a complex caseload, and respond flexibly to operational needs

Must be based locally or willing to commute daily across Barnsley for visits at short notice

Additional Information:

This role requires you to be available daily for community and hospital visits

Hybrid working policy in place but requires physical presence in the borough for urgent tasks
