About the Role
The London Borough of Hackney is seeking a highly skilled and committed Senior Practitioner to join our dynamic Adults Commissioning Reviews Team.

This is a vital role focused on ensuring that commissioned placements continue to meet the assessed needs of individuals, maintain high standards of care, and deliver value for money.

Key Responsibilities
Undertake comprehensive reviews of individuals placed in commissioned services across the borough.

Ensure that all reviews cover six critical elements:

Ongoing assessment of individual needs.

Service user voice - placing the views of individuals, families, and representatives at the heart of the process.

Transfer and transitions - facilitating changes to alternative services where required.

Quality assurance - ensuring that care is consistently high-quality.

Safeguarding - services must protect and promote the well-being of individuals.

Cost-effectiveness - services must offer value for money and align with council objectives
