Role: Social Worker - Adults

The Adults Locality Teams at BANES Council are currently looking for an experienced Social Worker to support the delivery of high-quality assessments and care planning in line with the Care Act 2014.

This role is focused on reducing current waiting lists by carrying out timely Care Act assessments and reviews. The successful candidate will be required to travel across the BANES area and work partially office-based, depending on service needs.

? Key Responsibilities:
Undertake Care Act assessments with adults in the community

Develop person-centred care and support plans

Liaise with health professionals, providers, and families

Contribute to reducing caseload backlogs within the locality teams

Work flexibly between community visits and office-based duties
