Cambridgeshire County Council is seeking an experienced Qualified Social Worker (QSW) to join our Transfers of Care and Continuing Healthcare (CHC) team at Hinchingbrooke Hospital. This is a hybrid role requiring face-to-face assessments on-site (potentially daily) alongside remote working.

As a Senior Social Worker, you will take a lead role in managing complex hospital discharges and CHC processes while supervising and supporting other social work staff.

Key Responsibilities
Undertake person-centred social care assessments and care planning to facilitate timely and safe hospital discharge

Apply strengths-based approaches in line with the Care Act 2014

Collaborate with NHS colleagues to support CHC assessments and decision-making

Supervise and manage staff, including allocation of workloads and case oversight

Participate in team development, reflective practice, and inter-agency meetings

Requirements
Qualified Social Worker (Social Work England registered)

Extensive post-qualifying experience in hospital discharge and/or CHC settings

Strong knowledge of relevant legislation including the Care Act, Mental Capacity Act, and Discharge to Assess pathways

Previous experience in a senior or supervisory social work role is highly desirable

Must be available for face-to-face work at Hinchingbrooke Hospital
