Manchester - QSW - Hospital discharge to assess Hybrid
Ref: 3430302034_1753181576Manchester City Council is seeking an experienced Qualified Social Worker to join the Hospital Discharge to Assess South Team (Wythenshawe area).
Role Overview:
You will support hospital discharges by completing Care Act assessments, Mental Capacity assessments, Best Interest decisions, and safeguarding work. This role is vital in reviewing care packages within weeks of discharge and ensuring appropriate long-term support is in place before cases are transferred to the community team.
Requirements:
Qualified Social Worker (SWE registered)
Solid experience in D2A, hospital discharge, and adult social care
Strong knowledge of the Care Act, MCA, Best Interests, and safeguarding
Ability to complete timely assessments and reviews
Hybrid working model - flexibility to work remotely and attend site as needed
Rate: £30 per hour
Location: Wythenshawe, Manchester (Hybrid)
