Lancashire County Council is seeking an Experienced Qualified Social Worker (QSW) to join their Adult Social Care Transition Team. This is a vital role supporting young people as they transition into adult services, ensuring a person-centred and legally compliant approach to care planning and delivery.

Key Responsibilities:

Manage a caseload of complex transition cases

Complete Care Act assessments, MCA assessments, and support planning

Contribute to safeguarding processes and risk management

Engage in multi-disciplinary working with health, education, and legal services

May involve Court of Protection work

Record and manage case information via the LAS system

Ideal Candidate:

Previous experience working within Lancashire County Council

Strong understanding of LBLIL processes

Skilled in using LAS (Liquidlogic Adults System)

Confident working autonomously and as part of a team

Experienced in complex casework and legal frameworks relating to adult social care
