Job Title: Social Worker - Mental Health Acute Hospital Discharge
Location: Stoke-on-Trent
Rate: £38 per hour (umbrella)
Working Pattern: Minimum 3-4 days on-site some flexibility for remote working based on service needs
Contract Type: Locum

We are currently recruiting for an experienced Mental Health Social Worker to join the Acute Hospital Discharge Team in Stoke-on-Trent.

This is a fast-paced role focused on supporting timely and safe discharges from acute mental health settings. The successful candidate will carry out assessments under Section 117 and the Care Act, ensuring appropriate care and support planning.

Key Requirements:

Experience in acute hospital discharge within mental health

Strong understanding of Section 117 and Care Act assessments

Sound safeguarding knowledge

Proficient in using Liquid Logic (case management system)

Able to work 3-4 days a week on-site, with flexibility for some remote work
