North Lincolnshire Council is seeking a skilled and dynamic Occupational Therapist to join their Integrated Discharge Team. This is a crucial role focused on ensuring safe and timely hospital discharges, supporting individuals to return home or to appropriate settings with the right equipment, adaptations, and care in place.

You will work as part of a multi-disciplinary team alongside health and social care colleagues, supporting discharge planning and reducing delays. The position offers flexible hybrid working with both onsite and community-based responsibilities.

Key Responsibilities:
Conduct functional assessments to support safe and effective hospital discharges

Prescribe and coordinate equipment and minor/major adaptations

Work closely with patients, families, and health professionals to create person-centred discharge plans

Complete manual handling risk assessments

Liaise with housing, care providers, and contractors where adaptations are required

Support DFG (Disabled Facilities Grant) processes where relevant

Maintain accurate and timely documentation in line with professional standards

Contribute to hospital avoidance strategies where possible

Essential Criteria:
Degree or Diploma in Occupational Therapy

HCPC registration

Proven experience in hospital discharge, reablement, or community-based OT work

Knowledge of moving and handling equipment and techniques

Experience with housing adaptations and DFG processes is advantageous

Strong understanding of relevant legislation (Care Act, Manual Handling, etc.)

Enhanced DBS (must be in place prior to start)

Ability to work independently and as part of a fast-paced team

Car driver with access to a vehicle
