Job Title: Qualified Social Worker - Transition Team
Location: Brent
Contract: Full-time, Permanent
Salary: Competitive (Depending on Experience)

Brent Council is looking for an experienced Qualified Social Worker (QSW) to join our Transition Team. This role focuses on assessing the needs of children and young people as they transition into adulthood and ensuring they receive the necessary support to live independently.

Key Responsibilities:
Assess the needs of children and young people transitioning to adulthood

Complete Care Act assessments, Child and Family assessments, Transition assessments, and Mental Capacity assessments

Submit Deprivation of Liberty Safeguards (DoLS) applications

Present cases at Resource Panel to ensure appropriate funding and support

Liaise with multi-disciplinary teams (MDT) and external agencies to provide comprehensive care and support

Take part in the duty rota and support with urgent cases as required
