Job Title: Mental Health Discharge Liaison Social Worker
Location: London Borough of Haringey
Rate: £32.52 p/h
Contract: Temporary

Overview:
An exciting opportunity has arisen for a Mental Health Discharge Liaison Social Worker within the London Borough of Haringey. The role will involve conducting assessments, developing support plans, and ensuring individuals receive the appropriate services and resources to meet their needs.

Key Responsibilities:

Conducting assessments and developing tailored support plans.

Ensuring access to the right services and resources for individuals.

Collaborating closely with healthcare professionals and external agencies.

Attending formulation meetings and working with the Mental Health Social Care Locality Social Worker.

Supporting clients who are open to services and readmitted to hospital.

Requirements:

Expertise in mental health legislation and a thorough understanding of the social care system.

Ability to collaborate effectively with healthcare professionals and agencies.

Experience in conducting assessments and creating support plans.

Strong interpersonal and communication skills.
