An exciting opportunity has arisen for an experienced Qualified Social Worker (QSW) to join the Hospital Discharge to Assess (HDTA) Team in Manchester.

This hybrid role is ideal for social workers with hospital discharge experience who are looking for flexible working while making a direct impact on patients' transitions from hospital into community settings.

Key Responsibilities:
Undertake assessments and discharge planning in line with the Care Act 2014 and D2A model.

Collaborate with health professionals to enable timely, safe hospital discharges.

Carry out strength-based assessments and ensure appropriate support packages are in place.

Participate in a 1 in 5 weekend rota.

Attend the hospital for 1-3 duty days per week remaining days can be worked remotely.

Maintain timely and accurate case records.

Requirements:
Qualified Social Worker with Social Work England registration

Experience in hospital discharge/HDTA settings preferred

Strong understanding of relevant legislation (Care Act, Mental Capacity Act, etc.)

Ability to work independently in a fast-paced environment

Non-drivers will be considered
