We are seeking an experienced and motivated Qualified Social Worker (QSW) to join the Hospital Discharge Team in Darlington. This is an excellent opportunity to work in a collaborative, multi-agency environment supporting individuals post-discharge from both acute and community hospitals.

Role Overview:
Work within a team of Social Workers, Community Assessment Officers, and an in-house Reablement Service.

Respond to hospital discharges across local acute and community hospitals in the Darlington area.

Visit individuals in hospitals, care homes, and their own homes to carry out person-centred assessments and planning.

Complete Care Act assessments, support planning, and initiate safeguarding enquiries as needed.

Record and consider Mental Capacity Act (MCA) implications within assessments and decisions.

Monitor and review post-discharge progress, ensuring individuals continue to improve in their reablement or recovery journey.

Work from the Hundens Lane base and report directly to the Team Manager.

Requirements:
Social Work England registered

Strong experience in hospital discharge planning, reablement, and care and support assessments

Understanding and application of the Care Act, MCA, and safeguarding principles

Ability to travel around Darlington to various care settings

Commitment to a Home First approach and strength-based practice
