An exciting opportunity has arisen to join the Adult Social Services Team at the Norfolk and Norwich University Hospital.

This role is perfect for an experienced Social Worker interested in complex casework within a fast-paced, multi-disciplinary hospital environment. You will work closely with health professionals to support safe patient discharges and deliver high-quality social care.

Key Responsibilities:

Complete Care Act Assessments and Mental Capacity Assessments

Lead on Discharge to Assess planning and Safeguarding enquiries

Collaborate with a wide multi-disciplinary team to ensure timely and safe hospital discharges

Manage a varied and dynamic caseload

Requirements:

Qualified Social Worker (BA, MA, DipSW, or CQSW)

Registered with Social Work England

Able to work on-site at the hospital (office-based role)
