Job Description

Serco is currently recruiting for a full time Community Care Officer (CCO). As a CCO you will be a key member of the service providing advice and guidance to citizens, undertaking assessments in the community. Using person-centred approaches you will produce holistic solutions to problems and challenges encountered by our clients in their daily living. Providing support, advice and assisting in finding appropriate community and universal services.

You will assess citizens who may require Adult Care and Support. Assist in the provision of advice and information to citizens, and other stakeholders, effectively identify and report safeguarding incidents and recognise need for Mental Capacity Assessments and be risk aware.

You will be required to maintain accurate written and electronic records and performance information whilst working within an equality of opportunity framework, recognising difference and diversity and to conduct themselves in an appropriate anti-discriminatory manner

To be successful in the role you will show confidentiality in all aspects of the work relating to customers and patient detail whether verbally or by electronic systems.

Candidate Description

A full driving licence is essential for this role as you will be required to travel within the local community. You must have experience of working within a social care or care provider environment and have strong use of IT systems and the customer information system.

Due to the nature of the role you must be willing to undertake generic and specialist training as required and maintain an ongoing commitment to continuing professional development (CPD).
